This office will be offering the service of ‘living wills’, formally known as advance medical directives, whereby a person sets out their wishes regarding medical treatment if they ever become incapable of consenting.
The procedure is as follows:
1. The client, together with a medical practitioner of their choice, completes the Model Form of Advance Medical Directive;
2. The client presents the completed form to the notary for authentication and registration at the Public Registry; and
3. The client submits the authenticated form to the Office of the Director General for Health Services.
